It was folded as I have pictured with the cardboard inserts but out of the plastic. I did have to open it and give it a gentle hand soak. Short sleeves with collar, single breast pocket and the straight shirt tail. Label is Brent which was a Montgomery Ward label. 100% cotton. Labeled a size medium, 15 15 1 2.
